We are currently looking for an RN Care Manager to join our team.

The right candidate will be focused on excellent customer service, effective communication and a passion for improving patient quality of life. The successful candidate will possess at least 3 years of experience working closely with patients, excellent organizational and time management skills, and an ability to work well under pressure.

Job Summary

The RN Care Manager is responsible for organizing, advocating and providing care management and care coordination services to patients within the practice. The RN Care Manager works in close collaboration with the providers to identify and partner with patients to optomize health outcomes using motivational interviewing, goal setting and follow-up health coaching, and care coordination.

The RN Care Manager works with patients with co-morbidities, unmanaged chronic disease, poor self-care, polypharmacy, and high utilization rates as well as Medicare Wellness to improve health outcomes.
